Follow these steps before placing your order to ensure we can process your claim quickly:

Step 1: Verify Your Coverage (The Customer's Responsibility)

  • Coverage Check: Please contact your insurance provider (or check your plan booklet) to confirm that the specific item you wish to purchase (e.g., knee brace, walker) is covered under your plan.

  • Deductible: Check if you have any outstanding deductible amount that must be paid out-of-pocket first.

  • Pre-Authorization: For expensive or custom items (like power chairs), ask your provider if pre-authorization is required.

Step 2: Gather Required Documentation

For us to submit your claim successfully, you must provide the following before we can release your product:

Required Document Details
Prescription/Referral A valid prescription or written recommendation from a recognized prescriber (Doctor, Physiotherapist, etc.). It must include the item name, quantity, and your diagnosis.
Insurance ID Your current member ID number and group/policy number.
Assessment Report (If required by your insurer for complex items, such as a specialized seating assessment).

Step 3: Complete Your Purchase

  • In-Store: Bring all your documentation with you. We will process the claim in real-time.

  • Online/Phone: Email all documentation to contactus@calmedi.com. We will process the claim and call you to arrange payment for any uncovered balance before shipping.

    You are responsible for paying the remaining balance (the co-pay or deductible) at the time of purchase.

  • What if I have another provider (e.g., Blue Cross, Manulife)?

    For all providers not listed above, you will need to pay for your purchase upfront. We will provide a detailed, itemized receipt that includes all necessary codes (including the make and model number) for you to submit to your insurance company for easy reimbursement.

  • Can you submit a predetermination?
